Once you have scheduled a new or re-establish appointment, you should receive a link to complete our forms.  If, by chance, you don’t – please call or text us so we may get you a link prior to your appointment. 

Important Information For Patients of

Blatt & Kingston Family Dental

• In order to best serve you as a patient of Blatt & Kingston Family Dental please complete the Patient Forms.
• On the day of your appointment, please arrive 10 minutes before your scheduled appointment. It is required that you bring your Dental Insurance card with you.
• A parent or guardian must accompany patients under 18 years of age and remain at the office during the length of the appointment.
• Payment for dental services is due at the same time you receive the dental care. If you have dental insurance please refer to the Dental Insurance Information portion of the Patient Forms.
